We are located at 4915 42nd Avenue North in Robbinsdale, just west of the railroad tracks between Highways 100 and 81. We are on bus lines 14 and 52K. Call Bus Line at 612-373-3333 for specific instructions. If you find it extremely difficult to get transportation to the clinic, give us a call and we'll try to assist you.

The Annex sees people between the ages of 12 and 25 by appointment for the following services.

MISSION
The Annex's Mission:
To support the young people of Northwest Hennepin County in the challenging transition from adolescence to adulthood through advocacy, health care, counseling and education.
THE ANNEX'S VISION
The Annex will be a catalyst for creating a community where all young people have the resources and support they need to successfully make the transition from adolescence to adulthood.
HISTORY
Located in Robbinsdale, the Annex Teen Clinic was founded in 1971 through the joint efforts of Holy Nativity Lutheran Church and St. Raphael's Roman Catholic Church. The clinic's services were originally limited to testing and treatment for sexually transmitted diseases, with a strong emphasis on health education.
In later years, the Annex became an independent, non-profit agency, and services were expanded to include more comprehensive sexuality-related health services for adolescents and young adults. The Annex now provides low-cost, sexuality-related health care, health education, counseling and outreach to adolescents in North and Northeast Minneapolis and northwest suburban Hennepin County. All services are confidential and are offered free or on a sliding fee scale.

Who can be seen at the Annex?
We see people between the ages of 12 and 25-males and females-for medical and support services. When someone turns 26, we can refer them to another community clinic.
STAFF
A medical director supervises clinical services provided by a paid staff and more than 100 volunteer medical doctors, nurse practitioners (NPs), physician's assistants (PAs), nurses, health educators and counselors. The staff includes an executive director, clinic director, health education director, volunteer programs manager, public health nurse, health educator, nurse practitioner, staff nurses and lab technicians.
VOLUNTEERS
Volunteer Opportunities at the Annex:
Physician/Nurse Practitioner/Physician Assistant: Performs routine reproductive examinations, family planning, STI diagnosis and treatment. Requires experience in family planning and sexually transmitted infection prevention and treatment. Experience with adolescents helpful. Once per month, Tuesday or Thursday 6:30-9:30 p.m.
Nurse/Clinical Assistant: Assists with reproductive examinations, family planning and health education. Clinical experience helpful. Once per month, Tuesday or Thursday 6:15-9:30 p.m.
Intake Counselor: Provides intake interviews, takes social and medical history, provides pregnancy counseling, STI and pre-HIV test counseling, and health education. Requires bachelor's degree and experience as a nurse, social worker, counselor or educator. Once per month, Tuesday or Thursday 5:45-8:30 p.m.
Receptionist: Greets patients/clients, answers phones, provides appropriate forms to clients before their appointments. Requires respect for confidentiality, pleasant disposition, organizational skills and the ability to communicate with staff and other volunteers. Once per month, Tuesday or Thursday 5:15-8:30 p.m.
Birth Control Educator: Provides information on services, male and female anatomy, sexuality, decision-making and all methods of birth control in small group setting. Answers questions and encourages clients to examine and clarify their values and attitudes regarding their birth control choices. Once per month, Tuesday or Thursday 5:30-7:00 p.m.
Peer Educator/Youth Volunteer: Attends peer education training and helps staff health education booths at health fairs, etc. Helps with various tasks around the clinic. Flexible schedule.
Board Member: Attends monthly meetings, determines policy and oversees functioning of clinic. Serves on committees and active board for rapidly evolving organization.
FUNDING
The Annex is funded by client fees, individual donations, United Way, private foundations, state and county public health contracts and in-kind support from North Memorial Medical Center.
FAQs
Can I bring a friend or partner?
Yes! Friends and family members are always welcome. We especially encourage you to come with a friend or partner to attend the birth control class. We will, however, ask to speak to you alone for parts of your appointment.
Who provides services at the Annex?
Our services are provided by clinic staff and volunteers. Many of our providers (doctors, nurse practitioners and physicians' assistants), intake counselors, receptionists and nurses are volunteers who enjoy working with young people. You may see a different practitioner or nurse at each visit.
What does it cost to come to the Annex?
We charge for most services and supplies in order to keep our doors open. The fee for services is based on your age and/or income. We expect you to pay for any service, medication or supply you receive, but we are flexible about payment plans. If you can't pay when you come, we will still see you at the Annex. Please do your best to pay what you can. Please call for our current fees.
Can I use my insurance at the Annex?
Because we want to insure the confidentiality of our clients, we currently are not able to process insurance at the Annex. If you have insurance and can use it, it is usually better-and less costly-for you to go to a clinic that takes your insurance. (Someone has already paid for it!) Call your insurance company or talk to a staff person to help you figure this out. Be sure to ask about how they keep your medical information confidential.
What are my rights and responsibilities as a client at the Annex?
Your Rights at the Annex Teen Clinic
Confidentiality: No information about your visit to the Annex will be given to anyone unless you give us written permission.
Privacy: Conversations, exams, treatments and your medical chart will be kept as private as possible.
Information: We will give you the most accurate information possible. Please ask questions about anything.
Accept or Refuse Treatment:You may refuse any procedure or exam at any time. You may stop an exam at any time if you're not comfortable. If you do refuse, we will tell you about your options and consequences of refusing.
Complain: Please voice your opinions about the services you recieve at the Annex. If you have a concern about your clinic visit please talk with a staff person or call our executive director. We also welcome positive feedback!
Non-discrimination: We will not discriminate against anyone because of race, creed, religion, sexual orientation, disability, marital status, financial status, national origin, political beliefs or gender.
You have a responsibility to:
Give honest and complete information about your health and medical history.
Understand the health care choices and plans open to you. Ask questions if you don't understand.
Follow through with treatment plans that we have set up with you. For example, if you have a sexually transmitted infection, it is your responsibility to get treated and inform any partners that may also be infected.
Are the services at the Annex confidential?
All services at the Annex are strictly confidential. Information about you and your visit to the clinic is kept confidential and is not shared with others (including parents). We can only share information about you with people outside of the clinic with your written permission. We take this issue very seriously.

There are a few exceptions to this rule:
If you test positive for some STI's (such as chlamydia, gonorrhea or HIV), we are required by Minnesota state law to notify the MN Department of Health. They mainly use this information to keep statistics. (They won't call your parents, boy/girlfriend, etc.)
We are required by MN state law to report concerns about child abuse to Child Protection. (This is called mandated reporting.)
In an emergency, we might have to contact your emergency contact person.
In certain situations, we reserve the right to get you outside help if you tell us you are about to harm yourself or another person.
We use information contained in your medical record for statistical purposes, with the understanding that confidentiality will be maintained.
